For families who currently have access to DDA funds, we are excited to be able to accept DDA funds for our group classes and summer camps. To register for a class or summer camp using DDA funds, please make a note of the classes or camps for which you would like to register and click the “Apply Here” link. You will need the following information to submit the application:

• Student’s name and birthday

• Parent/caregiver name, phone number, email address and mailing address

• Case Resource Manager name, phone number and email address

• Names of camps or classes for which you would like to register

 

Submission of this form does not guarantee your enrollment in a camp or class. A Music Works NW staff member will contact you to inform you of the status of your registration status.  

 

Please note that if your student is enrolled in a camp or class using DDA funds, that you are committed to your student attending. Any illnesses or extenuating circumstances that prevent my student attending the camp must be communicated to MTProgram@musicworksnw.org in a timely manner.
